Vida Medical Clinic will offer free sports physicals for student athletes in August 2023.
Free sports physicals will be offered at Vida Medical Clinic (720 W. Association Dr., Appleton, WI 54914) on:
- August 1, 2023, 3-6 p.m.
- August 8, 2023, 3-6 p.m.
An appointment for a free sports physical may be made by calling Vida at 920-731-4354. Appointments are required.
A sports physical helps ensure the health, wellbeing, and safety of children before they participate in a sport. In Wisconsin, student athletes are often required to have a sports physical before participating in a sport. A sports physical includes a medical history and physical exam. Vida Medical Clinic is fully staffed with licensed medical providers, clinicians, and nurses, and our Vida’s nurse practitioner will administer the free sports physicals.
Students and their parent or guardian are asked to bring any necessary completed paperwork from their school/athletic department to the sports physical appointment with this, including the WIAA Sports Physical Form. A parent or guardian is required to be onsite at Vida during the appointment time.
All of Vida’s services are provided free of charge and are confidential. No insurance, proof of insurance, or income verification is required. Even if you have insurance, you can utilize our services, including a free sports physical. Vida can also provide medical records to your primary care provider or your medical home, at your request.
(Please note: A sports physical does not take place of the recommended “well-child” checks conducted by a family medicine or primary care provider at your medical home.)
