Sports Physical

A sports physical helps ensure the health, wellbeing, and safety of children before they participate in a sport. In Wisconsin, high school student athletes are often required to have a sports physical before participating in a sport. A sports physical includes a medical history and physical exam.

Free sports physicals are available year-round at Vida, and student athletes from any school or school district are welcome! Vida will also host numerous sports physical clinic days in summer 2024. What do I need to bring with me to my sports physical?

Please bring your completed, school-required paperwork with you, including the completed WIAA sports physical form. Please arrive 15 minutes prior to your scheduled appointment time. A parent or legal guardian must be at Vida during the child's sports physical.

How long will the sports physical appointment be?

A sports physical typically takes about 15 minutes to complete, but at Vida, we allot additional time for each appointment time, so every patient feels well cared for and has questions answered.

Who is performing the sports physicals?

Vida Medical Clinic is fully staffed with licensed medical providers, clinicians, and nurses. Our clinic's nurse practitioner will administer the free sports physicals.

My family has insurance. Can we still utilize this free sports physical service?

Yes! All of Vida's services are provided free of charge and are confidential. No insurance, proof of insurance, or income verification is required. Even if you have insurance, you can utilize our services. We are here to serve! Vida can also provide medical records to your primary care provider or your medical home at your request.
